Lebanese General Security

The requested documents:

Investigation report done by the Interior Security Forces

A certificate of birth that conforms to current regulations

A civil status extract for the child bearing his adoptive name

A document signed by the orphanage waiving the child

An agreement issued by spiritual religious court concerning the adoption

A marriage attestation procured by the adopting family

The approval issued by local governmental entities

A medical attestation that underlines the infertility of one of the spouses

An extract of judicial record for both the kid and the adoptive parents

A certified copy of the passport held by the adoptive parents

An attestation that underlines the fact that both the parents and the orphanage didn’t receive any form of revenue from the above mentioned procedure

The approval issued by the ministry of internal affairs and the municipality

An attestation of baptism for the adopted kid and the adoptive parents

An accreditation issued to the orphanage

A proxy done by one of the parents to the other, if one of them is unable to show up

NB:

The above-mentioned documents have to be authentic, and the copies should be certified

The procedures required in such a case are held in the general secretarial office of the general security, by the adoptive parents. The documents are then transferred to the bureau of passports, naturalization and foreign affairs.

Child born in special circumstances An individual registered under his mother’s surname, without a mention of his father’s true identity (considered anonymous), that conveys a request of obtaining a passport, has to respect the following criteria:

A passport for an adult applicant (hyperlink)

A passport for a minor: his mother should show up at the competent general security bureau, where she is interrogated, having in hand the requested documents, including the judicial decisions that led to the registration of the child under the mother’s name

The final decision is issued by the general director of the general security.
A citizen without a family name Citizens that do not have family names, have a right to a passport according to the information provided by the identity card or an extract of civil status
